The May 2026 GLP-1 pricing picture looks superficially similar to April but has shifted in three structurally meaningful ways.

First, the higher-dose tier is now established. Wegovy HD (semaglutide 7.2 mg), approved by the FDA in March 2026 and launched April 7, is shipping at $399/month self-pay and as low as $25/month for many commercially insured patients via the savings offer — pricing that sits between the standard Wegovy 2.4 mg and the next-generation incretins. Lilly's Foundayo (orforglipron) hit its fourth commercial week with 7,335 prescriptions per Citi's May 8 IQVIA note, with telehealth platforms accounting for roughly 35% of launch volume that the IQVIA retail tracker misses.

Second, the compounded-GLP-1 channel is closing. The FDA's April 30 proposal to strike semaglutide, tirzepatide, and liraglutide from the 503B outsourcing-facility bulks list opened a 60-day comment window that closes June 29. After final rulemaking, large-scale 503B compounding effectively ends. The 503A pathway remains until the July 23-24 PCAC meeting, but the trajectory is clear — most patients currently paying $159-$349/month for compounded versions will need to migrate to branded supply.

Third, the Medicare GLP-1 Bridge launches July 1. Eligible Part D beneficiaries will pay $50/month for Wegovy, Zepbound, and potentially Foundayo through December 31, 2027. State Medicaid programs began joining May 1 and can opt in through January 1, 2027. This is the first concrete government-payer GLP-1 program with a defined pricing structure, though restricted to specific eligibility criteria.

The bottom-line price points for May 2026: commercially insured patients still pay as low as $25/mo with a savings card. Uninsured self-pay starts at $149/mo (Foundayo LillyDirect starter, Wegovy pill 1.5 mg) and runs to $499/mo (Wegovy injectable NovoCare). Medicare patients pay $50/mo through TrumpRx and (starting July 1) the GLP-1 Bridge. WAC list prices haven't moved meaningfully and remain disconnected from what anyone actually pays.
