Icotyde (Icotrokinra) Pricing 2026: Psoriasis Pill at About $8,500 a Month vs Sotyktu, Otezla, and Shots
J&J's Icotyde runs about $8,500 a month at cash prices. How it compares with Sotyktu, Otezla, Tremfya, and Skyrizi, plus J&J's $0 copay and free-drug programs.
Icotyde is a branded prescription pill, not a compounded peptide. The FDA approved Johnson & Johnson's Icotyde (icotrokinra) on March 18, 2026, for moderate to severe plaque psoriasis in adults and in children 12 and older who weigh at least 40 kg (88 pounds). Icotrokinra is a peptide, a short chain of amino acids, built to block the receptor for IL-23, an immune signal that drives psoriasis. The leading drugs that target the same pathway, Tremfya and Skyrizi, are antibodies given by injection. Icotyde is one 200 mg tablet a day, taken with water on an empty stomach, at least 30 minutes before eating. In the four Phase 3 trials behind the approval, 57% to 71% of patients on the drug had clear or almost clear skin at week 16. Nothing in this guide is compounded or sold for research.
J&J has not published a list price, so the best figures come from analysts and pharmacies. When the drug was approved, CNBC reported that J&J had not said what it would cost. In July 2026, analysts at Citizens put Icotyde's monthly wholesale acquisition cost at $8,285, or about $99,400 a year. The wholesale acquisition cost, or WAC, is the list price a drugmaker charges wholesalers before discounts and rebates, and almost nobody pays it directly. Pharmacy prices line up with that estimate: on October 5, 2026, GoodRx listed an average retail price of $10,681.56 for 30 tablets and a coupon price of $8,490.03.
A few terms, defined once. A pharmacy benefit manager, or PBM, is the company that runs a health plan's drug coverage and negotiates rebates; CVS Caremark, Express Scripts, and Optum Rx are the largest. A formulary is the PBM's list of covered drugs. Prior authorization means your doctor must get the plan's approval before the pharmacy can fill the prescription. Step therapy means the plan makes you try a cheaper drug first. Icotyde is a specialty drug, so a specialty pharmacy fills it and ships it to your home.
On the analyst figure, Icotyde's list price sits closer to the injections than to the older pills. Bristol Myers Squibb's Sotyktu, another psoriasis pill, has a WAC of $7,135.55 for 30 tablets, and Amgen's Otezla lists at $5,589.89 for a month's supply. Tremfya lists at $14,566.44 per injection and Skyrizi at $23,838.42 per dose, which works out to roughly $95,000 and $103,000 a year after the starting doses. With insurance, most of these gaps shrink: every one of these drugs has a manufacturer copay program that can bring the cost to $0 for people with commercial coverage.

Icotyde list price and how it is sold
J&J did not announce a price at approval, and we found no list price on its websites. The $8,285 monthly figure comes from a Citizens analyst note reported on July 17, 2026, and pharmacy cash prices tracked by GoodRx sit a little above it. Icotyde ships from specialty pharmacies under the pharmacy benefit, the same part of your plan that pays for pills at the drugstore.

J&J savings and free-drug programs
J&J runs three programs through ICOTYDE withMe. The savings card is for people with commercial insurance and does not ask about income. A separate access program gives the drug free while a commercial plan delays or denies coverage. People who are uninsured, or whose coverage falls short, can apply to the Johnson & Johnson Patient Assistance Program, which does have income rules. The savings card cannot be used with Medicare, Medicaid, or other government coverage.

Insurance coverage so far
Coverage is decided plan by plan, but two large commercial players have published their rules. Express Scripts lists Icotyde as a preferred drug for inflammatory conditions on its 2026 National Preferred Formulary. UnitedHealthcare requires prior authorization, though its written criteria do not demand that you fail another drug first. We found no public Icotyde decision from CVS Caremark as of October 5, 2026. On Medicare, the 2026 Part D redesign caps what you pay out of pocket each year, if your plan covers the drug.

Oral competitors: Sotyktu and Otezla
Sotyktu (deucravacitinib) is a once-daily pill that blocks TYK2, an enzyme in the same immune pathway. Otezla (apremilast) is an older pill taken twice a day that works on a different target, an enzyme called PDE4. Bristol Myers Squibb now sells Sotyktu directly to cash-paying patients for $743 a month, far below its list price; insurance cannot be used with that offer.

Injectable IL-23 drugs: Tremfya and Skyrizi
Tremfya (guselkumab), also from J&J, and Skyrizi (risankizumab) from AbbVie are the antibodies Icotyde was built to rival. Both start with two doses four weeks apart. Tremfya then repeats every 8 weeks and Skyrizi every 12 weeks, so the first year costs more than later years. Copies of Stelara (ustekinumab), an older injectable that blocks IL-12 and IL-23, launched in 2025 at deep discounts.

How to estimate what you will pay
Start with the kind of coverage you have, because that decides which program applies. With commercial insurance, ask your doctor's office to check whether your plan covers Icotyde and requires prior authorization, then enroll in the savings card before the first fill. If your plan uses a copay accumulator, maximizer, or SaveOnSP, the card may be worth less or not usable, so call J&J at 844-494-8463 before you start. On Medicare, the card is off the table, but the 2026 Part D cap limits your yearly spending if your plan covers the drug. Without insurance, the Patient Assistance Program is the main route, since cash prices run above $8,000 a month.
