Compounded Tirzepatide and Semaglutide Recalls, GLP-1s for Knee Osteoarthritis, an Oncolytic Peptide From Johns Hopkins

FDA logs sterility recalls of compounded tirzepatide, semaglutide, and sermorelin, a review weighs GLP-1s for knee arthritis, and a tumor-bursting peptide.

Editor's Note

Friday's one regulatory item came from FDA's weekly enforcement report: two compounders, one in North Carolina and one in South Carolina, recalled tirzepatide, semaglutide, vitamin B12 blends, sermorelin, and other injectables because their sterility could not be assured. No illnesses were reported, and none of the products were brand-name Mounjaro, Zepbound, Ozempic, or Wegovy. A systematic review found the best evidence that GLP-1 drugs help knee osteoarthritis still rests on pain and function in randomized semaglutide data, with claims about cartilage and avoided joint replacements resting on observational data. The rest of the day was early-stage peptide science: a Johns Hopkins peptide that bursts cancer cells and cleared some tumors in mice, a computationally redesigned antimicrobial peptide that was less toxic than its parent, a sea anemone toxin that kills crop pests, and a database of more than 4 million peptide sequences pulled from public mass-spectrometry studies.

FDA Enforcement Report Lists Sterility Recalls of Compounded Tirzepatide, Semaglutide, B12 Blends, and Sermorelin From Two Carolina Compounders

FDA's October 14, 2026 enforcement report lists 18 Class II recall entries, classified October 8, for compounded injectables for lack of assurance of sterility. Apothecary Pharma of Cary, North Carolina recalled 22 multi-dose vials of tirzepatide 18 mg/mL and one NAD+ vial distributed nationwide, starting September 24; Carolina Infusion of Ridgeland, South Carolina recalled patient-specific syringes of tirzepatide, tirzepatide and semaglutide blended with cyanocobalamin (vitamin B12), semaglutide, sermorelin, and erectile-dysfunction mixes, citing processing equipment and, for some products, a formulation using bacteriostatic water. Fox station coverage on October 21 noted that no illnesses or injuries were reported and that FDA had sent Apothecary Pharma a warning letter in December 2025 over sterile manufacturing practices, without linking it to the recall.

Systematic Review Finds GLP-1 Drugs May Ease Knee Osteoarthritis Pain, but Evidence for Slower Joint Damage or Fewer Replacements Is Low-Certainty

A systematic review published in BMC Musculoskeletal Disorders on October 23, 2026 screened 910 abstracts and included two randomized trials and five observational studies covering 160,089 patients, too varied to pool. The strongest evidence came from randomized data: semaglutide improved WOMAC knee pain by 14.1 points more than placebo on a 0-100 scale, with better physical function. Observational studies linked GLP-1 use to fewer hip (hazard ratio 0.60) and knee (0.75) replacements and slightly slower cartilage loss, which the authors rate as low-certainty; they call for longer osteoarthritis-specific trials.

Johns Hopkins Oncolytic Peptide NF27 Bursts Cancer Cells, Draws Immune Cells Into Tumors, and Eradicated Some Tumors in Mice

Johns Hopkins University School of Medicine researchers reported in the Journal of Medicinal Chemistry on October 23, 2026 a structure-activity study of peptides derived from CKS1, identifying a proline hinge and the arrangement of water-loving and water-repelling amino acids as keys to how these peptides rupture cancer cell membranes. The lead, NF27, killed a broad range of cancer cells with little toxicity to healthy cells and low hemolysis, and the cell death it caused was immunogenic, drawing immune cells into mouse tumors. In mouse tumor models, NF27 suppressed growth and completely eradicated some tumors without observable systemic toxicity; no oncolytic peptide has been approved for clinical use.

Computational Redesign of the Snake Cathelicidin NA-CATH Yields NA-7, Less Toxic in Mice and Active Against Drug-Resistant Klebsiella

Researchers from the Chinese Academy of Sciences' Xinjiang Institute of Ecology and Geography and the Chinese Academy of Medical Sciences reported in ACS Omega on October 23, 2026 that NA-CATH, a cathelicidin peptide first found in the Chinese cobra, was the best of five antimicrobial peptides tested on clinical multidrug-resistant Klebsiella pneumoniae and Acinetobacter baumannii in the lab, but killed all treated wax moth larvae. Screening nearly 50,000 computer-designed variants produced NA-7, which kept antibacterial activity, improved survival of Klebsiella-infected larvae compared with the parent peptide, and caused lower acute toxicity and fewer kidney changes in mice. The work is preclinical.

Hainan Team Synthesizes Ed-79, a 37-Residue Sea Anemone Toxin That Kills Fall Armyworm and Mealworm Pests

Researchers at Hainan Medical University reported in Molecules on October 23, 2026 a new peptide, Ed-79, found by screening the transcriptome of the sea anemone Exaiptasia diaphana: 37 amino acids held together by three disulfide bonds, made by solid-phase synthesis and folded in three oxidative steps. It killed fall armyworm (Spodoptera frugiperda) and mealworm (Tenebrio molitor) larvae in a dose-dependent way, with LD50 values of 289.5 and 367.2 pmol/g. Computer docking predicts it binds the outer vestibule of the Shaker potassium channel; that binding mode has not been confirmed experimentally.

PeptidomicsDB Reanalyzes 151 Public Peptidomics Projects Into a Searchable Set of More Than 4 Million Unique Peptides

Chongqing Medical University researchers described PeptidomicsDB in Nucleic Acids Research on October 23, 2026, a free resource built by reanalyzing 151 public mass-spectrometry peptidomics projects covering 3,992 samples, including neuropeptide and immunopeptide studies, with a single workflow in PEAKS Studio 13.0. It holds more than 33 million peptide records representing over 4 million unique sequences, more than 400,000 of them resembling known bioactive peptides, along with 78,910 peptide records that differ in abundance between conditions. The database links each peptide to its precursor protein, sample, and source study.
