MOTS-c (Mitochondrial Open reading frame of the Twelve S-rRNA type-c) is a 16-amino-acid mitochondrial-derived peptide encoded within the 12S rRNA region of mitochondrial DNA. It is one of the key "mitokines" — peptides that signal from mitochondria to the rest of the body — and has become a cornerstone of longevity and metabolic health protocols.
Mechanism: MOTS-c activates the AMPK pathway (the same target as metformin), recruits PGC-1α for mitochondrial biogenesis, and translocates to the nucleus under metabolic stress to regulate gene expression. It reduces mitochondrial ROS and improves insulin sensitivity.
Clinical evidence: Preclinical studies show improved insulin sensitivity, glucose homeostasis, exercise capacity, and delayed pancreatic islet cell senescence. CB4211, a MOTS-c analog from CohBar, completed Phase 1a/1b trials showing favorable safety and positive trends in liver enzymes and glucose.
Standard protocol: 5–10 mg subcutaneous injection 2–3x per week for 8–12 week cycles. Commonly stacked with NAD+ and 5-Amino-1MQ for synergistic mitochondrial/NAD+ boost (BioLongevity Labs even sells a pre-blended longevity-stack vial).
Regulatory status (April 2026): NOT FDA-approved. MOTS-c was on the FDA's Category 2 list, but is among the 14 peptides expected to return to Category 1 per the February 2026 HHS announcement. The formal rule is pending FDA review meetings scheduled for July 2026 and early 2027. Insurance does not cover MOTS-c.
