The Six PCAC-Recommended Peptides: BPC-157, KPV, TB-500, MOTS-c, Semax, and Epitalon Pricing After the July 2026 Vote

On July 23 and 24, 2026, an FDA advisory panel said yes to six peptides. The Pharmacy Compounding Advisory Committee (PCAC) recommended adding BPC-157, KPV, TB-500, MOTS-c, Semax, and Epitalon to the Section 503A bulks list. It rejected a seventh, emideltide (DSIP), by a 6-7 vote, and the margins on the winners were thin: 8-6 with one abstention for BPC-157 and KPV, 7-4 for Epitalon. Every yes vote went against written advice from FDA career scientists.

The vote did not make any of these peptides legal to compound. Two terms explain why. Section 503A covers state-licensed pharmacies that make a drug for one named patient with a prescription. Section 503B covers larger "outsourcing facilities" registered with the FDA that can compound in bulk without patient-specific prescriptions. Either kind can only use an unapproved ingredient that appears on the matching bulks list, the FDA's roster of allowed substances. A PCAC vote is advice, and the FDA still has to publish a proposed rule, take public comments, and issue a final rule. As of September 2, 2026, the FDA's 503A bulks list page (last updated May 14, 2026) lists none of the six, and no proposed rule has appeared. HHS Secretary Robert F. Kennedy Jr. has to sign off on any addition. The next PCAC peptide meeting, on five different peptides, is due before the end of February 2027. The White House named Dr. Heidi Overton as its FDA commissioner pick on August 18, and her Senate review is pending.

So the market looks much like it did in April. Telehealth programs still sell these peptides as compounded prescriptions. Prices run from $80 a vial plus a $39 visit at RxPepsDirect to $445 per eight-week cycle at Perfect B in Doral, Florida. Research vendors still sell "research-use-only" vials: powder with a not-for-human-use label and no prescription, from $35 (Bluum TB-500, 5 mg) to $130 (Evolve Epithalon, 50 mg). Hims & Hers, whose chief medical officer testified at the meeting, says it will not sell any of the six until the FDA finishes rulemaking. A July 28 vendor survey found research-vial prices per milligram unchanged by the vote. A few pages did move between our April guides and this check. BeyondMD's BPC-157/TB-500 capsule program went from $199 to $249. Bluum's TB-500 fell from $50 to $35, and we cannot tie either change to the vote.

How to read the tables. Each peptide section lists a telehealth or clinic program, a compounded price where a program publishes one, and research vendors. Compounded prices sit in a gray zone: the sellers describe 503A pharmacies filling patient-specific prescriptions, yet none of the six is on the 503A list. The individual guides carry more vendors and dosing detail: BPC-157, KPV, TB-500, MOTS-c, Semax and Selank, Epithalon, and the KLOW blend that packages KPV with BPC-157, TB-500, and GHK-Cu. The seven votes, July 23-24, 2026

PCAC is a 15-member advisory panel. It votes on whether an unapproved ingredient belongs on the 503A bulks list, and the FDA then decides whether to act. All six yes votes overrode FDA staff briefing documents from June 29-30 that recommended against every peptide on the docket. Emideltide was the only rejection.

Where the FDA process stood on September 2, 2026

A yes vote starts a rulemaking clock, and the FDA has to publish a proposed rule, take comments for 60 to 90 days, answer them, and publish a final rule with an effective date. Holland & Knight puts the usual span at 12 to 24 months. None of those steps had started by early September, which leaves any pharmacy filling these prescriptions today working outside the 503A list.

BPC-157

BPC-157 is a 15-amino-acid fragment of a stomach protein, sold for gut and tendon repair. It is the most widely marketed of the six and the one with a national telehealth pipeline waiting on the rule, since Hims & Hers says it will not sell it before final rulemaking. Prices here run from an $80 vial plus a $39 visit to a $445 clinic cycle.

KPV

KPV is a three-amino-acid fragment of the hormone alpha-MSH, studied for gut inflammation. Clinics rarely sell it alone. Most programs fold it into a BPC-157 combination or the four-peptide KLOW blend, and RxPepsDirect states outright that KPV is 'not sold on its own.' Standalone research vials are the cheapest of the six.

TB-500

TB-500 is a synthetic piece of thymosin beta-4, a protein involved in cell movement and repair. Clinics almost always pair it with BPC-157 as the 'Wolverine stack,' so most program prices below are for the pair. One vendor changed its own label: Simon's, which our April guide listed as a compounding pharmacy, now sells TB-500 as research-use-only and says it is not a compounding pharmacy.

MOTS-c

MOTS-c is a 16-amino-acid peptide encoded in mitochondrial DNA and marketed for metabolic health. PCAC reviewed it for obesity and osteoporosis and gave it the closest yes of the six, 7-5 with two abstentions. Research-vendor pricing is tight, $43 to $116 for 10 mg, while clinic programs run $199 to $299 a month.

Semax

Semax is a seven-amino-acid ACTH fragment developed in Russia, where Peptogen sells it as a nasal drop. PCAC recommended it for cerebral ischemia, migraine, and trigeminal neuralgia. US access splits three ways: Russian pharmacy imports for about 10 euros, compounded Semax/Selank sprays and injections for $150 to $445, and research vials from $63.

Epitalon

Epitalon (also spelled Epithalon) is a four-amino-acid peptide from the St. Petersburg Institute of Bioregulation and Gerontology, sold for sleep and 'anti-aging.' PCAC's 7-4 yes covered adult insomnia only. Clinic programs are scarce, so most of the spread here is research vendors, $38 to $130 depending on vial size.

Price movement since the vote

We looked for evidence that clinics or vendors repriced after July 24 and found little. A July 28 vendor survey put research-vial prices per milligram unchanged, and Hims & Hers, the largest would-be seller, is not selling any of the six. Several individual pages did move between our April guides and September 2, in both directions, and we cannot link those moves to the vote.
