Thymosin Alpha-1 (Tα1, thymalfasin) is a 28-amino-acid immune-modulating peptide originally isolated from thymus tissue. It is used for immune support, chronic infections, cancer adjuvant therapy, and autoimmune modulation.
Zadaxin — the branded version manufactured by SciClone Pharmaceuticals — is FDA-approved or equivalent in 35+ countries for chronic hepatitis B, chronic hepatitis C, and cancer adjuvant therapy. It is NOT FDA-approved in the US, but the FDA has granted orphan drug status for malignant melanoma, chronic active hepatitis B, DiGeorge anomaly, and hepatocellular carcinoma.
Regulatory status (April 2026): Thymosin Alpha-1 was placed on the FDA's Category 2 list in late 2023, restricting compounding. On February 27, 2026, HHS Secretary RFK Jr. announced that Tα1 was among the 14 peptides being moved back to Category 1, restoring legal compounding access. The formal FDA rule is pending, but 503A compounding with a physician's prescription is broadly considered permitted as of April 2026.
Standard dosing: 1.6 mg subcutaneous twice weekly (the approved Zadaxin hepatitis B protocol: 1.6 mg twice weekly for 26–52 weeks). Typical wellness cycling is 2–3 months on, 1 month off.

Insurance does not cover Tα1 for off-label/wellness use in the US.
