Semaglutide Linked to 18% Lower Mortality Than Dulaglutide or Liraglutide in 3,913 Medicare Patients on Hemodialysis

Thomas Jefferson University researchers reported in Diabetes Care on October 27, 2026 a target-trial emulation using 2011-2022 U.S. Renal Data System and Medicare data on 1,312 semaglutide users and 2,601 dulaglutide or liraglutide users with diabetes on maintenance hemodialysis. After weighting, semaglutide was associated with lower risk of death on dialysis (subdistribution hazard ratio 0.82; one-year incidence 11% vs 13%) and all-cause hospitalization (0.91; 57% vs 62%), and more weight loss at one year (4.8% vs 3.4%). The advantage held against dulaglutide but was not statistically significant against liraglutide; the study is observational.

Review: Semaglutide Helped Some People on Dialysis Lose Enough Weight to Join Kidney Transplant Waitlists

Nephrologists at Zhejiang Chinese Medical University reported in BMC Nephrology on October 13, 2026 a systematic review of semaglutide in adults on dialysis, whose obesity can block kidney transplant listing. Across 7 main studies with 147 participants, exploratory pooling found an average weight loss of about 7.3 kg, and in two transplant-candidate groups 14 of 29 and 13 of 23 previously ineligible patients were activated or listed, with 7 and 8 later receiving transplants. The studies were small, mostly uncontrolled, and highly varied, so the authors treated the pooled numbers as exploratory.
