Reddit Reports on Gray-Market Retatrutide, What Causes 'Ozempic Face', Semaglutide Before Kidney Transplant

Reddit users on gray-market retatrutide report fatigue and appetite swings, a review explains 'Ozempic face', and semaglutide helps some on dialysis.

Wednesday's research looked at what happens to people taking GLP-1 drugs outside the neat conditions of a trial. A Penn team used language models to read Reddit posts from nearly 7,800 people who said they were taking unapproved retatrutide and found a symptom pattern unlike the trials, with fatigue, appetite swings, and sleep and heart-rate complaints as common as nausea, though nobody can confirm what those users actually injected. A dermatology review concluded that 'Ozempic face' is mostly what any large weight loss does to the face, about 7% of midface volume per 10 kg lost, not a drug-specific effect. A Chinese review of small studies found semaglutide helped some people on dialysis lose enough weight to get onto kidney transplant lists. Two mouse studies rounded out the day: a Feinstein Institutes peptide that clears an inflammatory signal eased kidney injury, and a Fudan University peptide-drug conjugate aimed at the cells that guard the blood-brain barrier eased a multiple sclerosis-like disease.

Penn Analysis of Reddit Posts From About 7,800 Gray-Market Retatrutide Users Finds Fatigue, Appetite Swings, and Sleep Problems as Common as Nausea

University of Pennsylvania and Boston Children's Hospital researchers reported in a JAMA Network Open research letter on October 14, 2026 that they used large language models to read 148,640 retatrutide-related Reddit posts and comments from 27 communities, posted from 2021 to 2025, and mapped symptoms from 7,823 users who described current use of the unapproved drug. The most common terms were increased appetite (17.1%), fatigue (15.2%), increased energy (12.4%), nausea (11.6%), and insomnia (6.1%), and 4.8% mentioned a faster heart rate, a profile in which stomach symptoms were less dominant than in Lilly's trials. The authors called the findings hypothesis-generating, since what users injected, how much, and with what else cannot be verified.

Dermatology Review Finds 'Ozempic Face' Tracks the Amount of Weight Lost, About 7% of Midface Volume per 10 kg, Not the Drug Itself

A review by a Chung-Ang University dermatologist, published October 14, 2026 in Aesthetic Surgery Journal, pooled 46 reports on facial hollowing and sagging during GLP-1 weight loss. Serial imaging in 20 patients showed a median 9% loss of midface volume, more in superficial than deep fat (11% vs 7%), about 7% per 10 kg of weight lost, and a survey of 1,226 people linked more facial volume loss to greater percentage weight loss. Similar changes follow bariatric surgery, so the author concluded that weight-loss deflation on top of normal aging, not a drug-specific effect, explains most of it, and that evidence for cosmetic treatments is limited.

Review: Semaglutide Helped Some People on Dialysis Lose Enough Weight to Join Kidney Transplant Waitlists

Nephrologists at Zhejiang Chinese Medical University reported in BMC Nephrology on October 13, 2026 a systematic review of semaglutide in adults on dialysis, whose obesity can block kidney transplant listing. Across 7 main studies with 147 participants, exploratory pooling found an average weight loss of about 7.3 kg, and in two transplant-candidate groups 14 of 29 and 13 of 23 previously ineligible patients were activated or listed, with 7 and 8 later receiving transplants. The studies were small, mostly uncontrolled, and highly varied, so the authors treated the pooled numbers as exploratory.

Feinstein Institutes' Opsonic Peptide MOP3 Cuts Kidney Injury Markers After Blood-Flow Loss in Mice

Researchers at the Feinstein Institutes for Medical Research reported in Shock on October 13, 2026 that MOP3, a peptide derived from the protein MFG-E8 that tags the inflammatory signal eCIRP for removal by immune cells, reduced kidney damage when given as blood flow returned after 30 minutes of kidney ischemia in mice. Compared with untreated mice, MOP3 lowered circulating eCIRP by 40%, serum creatinine by 63%, blood urea nitrogen by 56%, and kidney injury markers such as KIM-1, and reduced tubular damage and cell death. The work is in mice.

Fudan Team's Pericyte-Targeting Peptide-Drug Conjugate Nanoparticle Protects the Blood-Brain Barrier in Mouse Models of Multiple Sclerosis

Researchers at Fudan University reported in ACS Nano on October 13, 2026 a peptide-drug conjugate that links a two-part targeting peptide to luteolin, a plant compound, packaged in nanoparticles that release it in response to oxidative stress. Aimed at pericytes, the cells that support blood-brain barrier vessels, it reduced barrier leakage and immune-cell infiltration and promoted myelin repair in two mouse models of multiple sclerosis, and injection under the skin every other day raised brain exposure 2.65-fold and beat daily intravenous dosing. The results are from mice.
