AOD-9604 is a synthetic fragment of human growth hormone (GH), 16 amino acids long, covering residues 177–191, the C-terminal region that drives fat breakdown (lipolysis). One tyrosine swap at the N-terminus makes it more stable. Metabolic Pharmaceuticals developed it and first studied it for obesity. It mimics the fat-burning action of full GH without three of GH's side effects: higher blood sugar, higher IGF-1, and tissue growth.
Clinical history: A Phase IIa trial (300 subjects, 12 weeks) tested a 1 mg dose, and patients lost 2.8 kg on average versus 0.8 kg on placebo (p < 0.05). A Phase IIb trial (536 subjects, 24 weeks) found about 2.6 kg of loss versus about 0.8 kg on placebo but missed its primary endpoint. Metabolic Pharmaceuticals ended development in 2007, and the safety record stayed clean across all trials.
Regulatory status (April 2026): AOD-9604 sits on firmer regulatory ground than most compounded peptides. FDA placed it on Category 2 in September 2023, the list of substances the agency considers too risky to compound while under review. It came off that list on September 27, 2024, because the original nominator withdrew, 17 months before the broader February 2026 announcement by RFK Jr. Today, licensed 503A and 503B compounding pharmacies may prepare AOD-9604 with a valid prescription. A 503A pharmacy compounds patient-specific prescriptions, while a 503B outsourcing facility compounds in bulk for clinics and hospitals. The drug remains not FDA-approved for any use.
Standard protocol: 300 mcg a day injected under the skin (subcutaneous). If weight loss stalls by week 4, the dose rises to 500–600 mcg a day, split between morning and bedtime, and trials showed doses up to 1,000 mcg a day were safe. Timing matters: take it fasted in the morning, because insulin released after a meal blocks AOD-9604's fat-burning action. A typical cycle is 12 weeks.
Stacks and coverage: Compounding pharmacies often pair AOD-9604 with GLP-1 drugs (semaglutide, tirzepatide) or blend it with MOTS-c and Tesamorelin in "fat burner" mixes. Insurance does not cover AOD-9604.
