AOD-9604 is a 16-amino-acid synthetic fragment of human growth hormone (residues 177–191, the C-terminal lipolytic domain) with a tyrosine substitution at the N-terminus for stability. Developed by Metabolic Pharmaceuticals and originally studied for obesity, it mimics the lipolytic action of full growth hormone without the hyperglycemic, IGF-1-elevating, or tissue-proliferative side effects of full GH.

Clinical history:

  • Phase IIa (300 subjects, 12 weeks): 1 mg dose produced 2.8 kg mean weight loss vs 0.8 kg placebo (p < 0.05)
  • Phase IIb (536 subjects, 24 weeks): ~2.6 kg loss vs ~0.8 kg placebo; primary endpoint NOT met
  • Development terminated by Metabolic Pharmaceuticals in 2007; safety profile remains clean across all trials

Regulatory status (April 2026): AOD-9604 has a better regulatory posture than most compounding peptides. Originally placed on FDA Category 2 (September 2023), it was removed from Category 2 on September 27, 2024 — 17 months before the broader February 2026 RFK Jr. announcement — because the original nominator withdrew. Licensed 503A/503B compounding pharmacies may legally prepare AOD-9604 with a valid prescription today. It remains NOT FDA-approved for any indication.

Standard protocol: 300 mcg/day subcutaneous, escalating to 500–600 mcg/day (split AM + pre-bed) at week 4 if plateaued. Up to 1,000 mcg/day shown safe in trials. Timing matters: morning fasted dosing is essential — post-meal insulin inhibits AOD-9604's lipolytic mechanism. Typical cycle is 12 weeks.

AOD-9604 is commonly stacked with GLP-1s (semaglutide, tirzepatide) or combined with MOTS-c and Tesamorelin in "fat burner" blends at compounding pharmacies. Insurance does not cover AOD-9604.
