GLP-1 Drug Pricing Guide: Best Prices as of June 2026

June 2026 GLP-1 pricing update: CVS Caremark Foundayo + Zepbound formulary deal, Wegovy pill UK + UAE launches, 503B sunset June 29, Medicare Bridge July 1.

The June 2026 GLP-1 pricing picture has shifted in three big ways since the May update.

First, CVS Caremark closed the PBM coverage gap. A PBM, or pharmacy benefit manager, decides which drugs an insurance plan covers and at what copay. Effective June 1, 2026, Caremark removed its new-to-market block on Lilly's Foundayo (orforglipron). Effective October 1, 2026, Zepbound returns to Caremark's commercial formularies (the covered-drug lists) as an additional preferred option. Caremark had dropped it in mid-2025. Express Scripts and OptumRx already cover both drugs, so all three major US PBMs are now aligned. The Caremark standard formulary template covers roughly 25-30 million Americans. CVS officials cited a price-secure agreement with Lilly that brings out-of-pocket cost as low as $25/month for eligible commercially insured patients.

Second, Novo Nordisk's Wegovy pill went international. The Emirates Drug Establishment cleared the UAE launch on June 3, 2026. Abu Dhabi and Dubai pharmacies stock the 1.5, 4, 9, and 25 mg strengths at AED 779-1,610 per month. The UK drug regulator, the MHRA, approved oral semaglutide 25 mg for chronic weight management on June 11. That is the third national approval, after the US (January 5) and the UAE. UK private prescriptions are expected within weeks. NHS coverage waits on a separate cost-effectiveness review by NICE, the body that decides what the NHS pays for. In the US, Wegovy pill prescriptions have crossed 3 million in the first five months on the market.

Third, the regulatory countdown is now days, not weeks. The FDA's comment window on excluding semaglutide, tirzepatide, and liraglutide from the 503B bulks list closes June 29, 2026, 16 days from today. (503B facilities compound drugs in bulk; 503A pharmacies fill patient-specific prescriptions.) The Medicare GLP-1 Bridge launches July 1 with a $50/month copay for eligible Part D members. PCAC, the FDA's Pharmacy Compounding Advisory Committee, stops taking oral-presentation requests June 30. Its July 23-24 meeting will weigh BPC-157, KPV, TB-500, MOTs-c, DSIP (Emideltide), Semax, and Epitalon. And Novo Nordisk's $199/month Wegovy injectable starter promotion (0.25 and 0.5 mg doses) also expires June 30. After that, starter doses revert to the standard $349 self-pay tier.

The bottom-line price points for June 2026: commercially insured patients still pay as low as $25/mo with a savings card or PBM coverage. Uninsured self-pay starts at $149/mo (Foundayo LillyDirect starter, Wegovy pill 1.5 mg, and Wegovy pill 4 mg through August 31) and runs to $449/mo (Zepbound prefilled pens, maintenance doses). Medicare patients pay $50/mo through the GLP-1 Bridge starting July 1. WAC list prices (Wholesale Acquisition Cost, the manufacturer's sticker price) have not moved and have little to do with what patients pay.

CVS Caremark Coverage Expansion — Foundayo June 1, Zepbound October 1

On May 28, 2026, CVS Caremark announced two coverage changes to its standard commercial formulary template, which covers roughly 25-30 million Americans. Effective June 1, 2026, the new-to-market block on Foundayo (orforglipron) came off. Effective October 1, 2026, Zepbound returns as an additional preferred option. Caremark had removed it in mid-2025, which pushed patients toward Wegovy. CVS cited an agreement with Lilly that brings out-of-pocket cost as low as $25/month for eligible commercially insured patients on both drugs. Each employer or union sets the actual copay tier.

Wegovy Pill International — UAE June 3, UK MHRA June 11

Novo Nordisk's Wegovy pill (oral semaglutide tablets) went international in June. The Emirates Drug Establishment cleared the UAE launch on June 3, 2026, and pharmacies stock all four strengths (1.5, 4, 9, 25 mg). The UK MHRA approved oral semaglutide 25 mg for chronic weight management on June 11. The approval rests on the Phase 3 OASIS 4 trial, which showed 13.6% weight loss at 25 mg versus 2% on placebo. UK private prescriptions are expected within weeks. NHS coverage needs a separate NICE cost-effectiveness review. US prescriptions have crossed 3 million in the five months since the January 5 launch.

Foundayo (Orforglipron) — Week 9 Tracker and CVS Caremark Open Coverage

Eli Lilly's oral GLP-1 pill, FDA-approved April 1, 2026 and launched April 7. The IQVIA tracker counted 16,982 prescriptions at Week 8. The real patient count is higher, because retail-only data misses much of the telehealth volume (around 35% of launch volume across 12+ platforms). With CVS Caremark's block gone as of June 1, Foundayo now has formulary access at all three major US PBMs.

Wegovy Injectable — $199 Starter Promotion Closes June 30, Standard Tier Returns

Novo Nordisk's injectable semaglutide for chronic weight management. The $199/month self-pay promotion on 0.25 mg and 0.5 mg starter doses ends June 30, 2026. After that, starter doses go back to the standard $349/month NovoCare self-pay tier. The 1.5 mg and 4 mg Wegovy pill doses are also $149/month through August 31, after which the 4 mg dose moves to $199. Wegovy HD 7.2 mg has its own $399/month self-pay tier. Wegovy injectable Q1 2026 sales hit DKK 12.7B (about $1.85B) on roughly 2.2 million Q1 prescriptions.

Zepbound (Tirzepatide) — LillyDirect Vials and CVS Caremark Return October 1

Eli Lilly's tirzepatide injection for chronic weight management. The December 2025 LillyDirect vial price cut (to $299-$449/month from $349-$499) still holds. CVS Caremark's October 1 return as a preferred commercial-formulary option will be the largest US coverage shift for Zepbound this year. Caremark already covers Mounjaro, the same molecule approved for type 2 diabetes. The obesity indication has been the contested one.

Compounded Semaglutide & Tirzepatide — 16 Days to 503B Sunset

On April 30, the FDA proposed removing semaglutide, tirzepatide, and liraglutide from the 503B bulks list. The 60-day comment window closes June 29, 2026, 16 days from today. Once the rule is final, large-scale compounding through 503B outsourcing facilities effectively ends. The 503A path through licensed pharmacies stays open until the July 23-24 PCAC meeting. Telehealth platforms that ran on compounded supply (Hims & Hers, LifeMD, and others) have already moved to branded distribution since Q1 2026. Patients now on compounded versions will need to switch to branded supply through NovoCare, LillyDirect, PBM commercial coverage, or the Medicare Bridge starting July 1.

Medicare GLP-1 Bridge — 18 Days to Launch

The Medicare GLP-1 Bridge program launches July 1, 2026 with a $50/month patient copay and a Medicare-negotiated price of about $245/month. Coverage runs through December 31, 2027. State Medicaid programs began joining May 1, 2026 and can opt in through January 1, 2027. To qualify, patients need a BMI of 30+ with a related health condition, or 27+ with pre-diabetes or established cardiovascular disease. Wegovy, Zepbound, and Foundayo are confirmed on the formulary. The April 22 cancellation of the broader BALANCE pilot narrowed the program's scope.
