Wegovy Pill Pricing: September 2026

The Wegovy pill has four prices, and the cheapest one only lasts a month. Novo Nordisk's oral semaglutide tablet comes in 1.5, 4, 9, and 25 mg strengths, and every strength carries the same list price of $1,349.02 for a 30-tablet bottle. That list price is the WAC, or wholesale acquisition cost: what Novo charges wholesalers before any discount or rebate. Almost nobody pays it. Self-pay patients ordering through NovoCare Pharmacy pay $149 a month for 1.5 mg, $199 for 4 mg, and $299 for 9 mg or 25 mg. The 4 mg tablet was $149 through August 31, 2026, so September is the first month the second step costs $199.

Dose escalation is why the pill gets more expensive. Patients start at 1.5 mg and move up about once a month, to 4 mg, then 9 mg, then 25 mg, because the gradual increase limits nausea. The bill therefore climbs from $149 to $299 by month three. A cash patient who reaches 25 mg spends about $946 in the first four months, then $299 a month. Foundayo, Eli Lilly's rival pill, climbs a similar ladder: $149 at 0.8 mg and $299 at 9 mg and above.

Insurance and government channels change the arithmetic. If an employer plan covers the pill, Novo's savings card lowers the copay to as little as $25 a month, although the card contributes at most $100 toward the difference. Whether the plan covers it is decided by a PBM, or pharmacy benefit manager, such as CVS Caremark or Express Scripts. The PBM maintains the formulary, the list of covered drugs, and usually requires prior authorization, a form the prescriber files to show the patient meets the BMI criteria. Medicare has its own path. The Medicare GLP-1 Bridge, live since July 1, 2026, charges a flat $50 copay for all four tablet strengths. Under the November 2025 agreement with the Trump administration, Novo and Lilly set a $245 monthly Medicare price for their injectables and committed to $150 starting doses for the pills. TrumpRx, the government's direct-purchase site, lists the pill at the same $149 and $199 as NovoCare.

Novo says the low price is deliberate. On August 5, 2026, CEO Mike Doustdar told CNBC the company "would not be able to show a positive growth on the top and the bottom if items like the pill were not doing well and were not profitable," and described the price as a "sweet spot" between price and volume. The volume is real: the pill passed 5 million prescriptions since its January 5 launch, and Novo reports that about 80% of pill patients had never used a GLP-1 before. IQVIA counted roughly 172,000 weekly US prescriptions in early August against 34,219 for Foundayo. The trade-off showed in Novo's gross margin, which fell to 78.2% from 82.7%.
