HCG & Gonadorelin Pricing: April 2026

Gonadorelin runs $15-$50/mo via compounding pharmacies and telehealth TRT clinics; branded HCG (Pregnyl, Novarel) runs $180-$430/vial with persistent supply issues.

HCG (human chorionic gonadotropin) and gonadorelin are the two most commonly prescribed adjuncts to testosterone replacement therapy (TRT). Both are used to preserve testicular function, maintain fertility, and prevent testicular atrophy that often occurs when exogenous testosterone suppresses the body's natural HPG axis.

Mechanism of action — the key difference:

  • HCG acts downstream: it directly mimics luteinizing hormone (LH), binding to LH receptors on the Leydig cells of the testes and stimulating testosterone and sperm production even if the pituitary is suppressed.
  • Gonadorelin acts upstream: it is a synthetic GnRH decapeptide that stimulates the pituitary gland to release LH and FSH, which in turn signal the testes. This more closely mimics the body's natural pulsatile hormone rhythm.

Regulatory status (April 2026):

  • HCG was reclassified as a biologic under the BPCIA transition on March 23, 2020, making it ineligible for 503A or 503B compounding. It is now only legally available through FDA-approved branded products (Novarel, Pregnyl). Supply has been inconsistent since 2020, with ongoing shortages reported through 2026 as only a handful of 503B facilities retain access to the approved brand supply. Only ~5 of 81 FDA-approved 503B pharmacies provide HCG as of the latest survey.
  • Gonadorelin is on the FDA 503A Category 1 bulk drug substances list, meaning it is available for compounding with a valid prescription. Its original FDA-approved brands (Factrel, LutrePulse) were discontinued, but 503A/503B compounders can legally prepare it. Following HCG's compounding restrictions, gonadorelin has become the dominant TRT adjunct in the US.

Typical dosing:

  • HCG: 250-500 IU subcutaneously 2-3 times per week (TRT adjunct); up to 1,500-3,000 IU 2x/week for fertility restoration
  • Gonadorelin: 100-200 mcg subcutaneously 2-3 times per week (common TRT protocol); some protocols use microdoses 2x/day to mimic natural pulsatility

Insurance: Rarely covers either medication for TRT/hypogonadism adjunct use. HCG has FDA-approved indications for female infertility and male prepubertal cryptorchidism, so insurance coverage is more likely when prescribed for those conditions. Most TRT patients pay out-of-pocket; HSA/FSA eligible with prescription.

Branded HCG (FDA-Approved, Pharmacy-Dispensed)

The only legal route for HCG in the US since the 2020 BPCIA biologic reclassification removed it from the 503A compounding pathway. Both Pregnyl (Organon) and Novarel (Ferring) are FDA-approved branded products. Both have experienced ongoing supply shortages since 2022.
