Mounjaro Pricing: September 2026

Mounjaro's list price rose to $1,112.16, and a $499 self-pay option arrived. The wholesale acquisition cost (WAC) is the list price a drugmaker sets, and for a 28-day fill of four Mounjaro single-dose pens it is now $1,112.16, according to the TrumpRx.gov listing. In April 2026 this guide cited a list price near $1,080. Eli Lilly now sells Mounjaro directly for self-pay through LillyDirect, where every dose from 2.5 mg to 15 mg is $499 a month. TrumpRx.gov shows the same $499 as its price and routes buyers to LillyDirect, which is a real change from a year ago, when Mounjaro had no cash-pay program at all.
Zepbound is the same drug at a lower self-pay price. Mounjaro and Zepbound are both tirzepatide, with Mounjaro carrying the type 2 diabetes label and Zepbound carrying the obesity label. Zepbound's LillyDirect vials and KwikPen cost $299 to $449 a month, while Mounjaro's self-pay pen costs $499. The label matters for insurance rather than for the molecule, so patients paying cash should ask their prescriber which label fits their diagnosis.
Insurance is where most Mounjaro patients land. Type 2 diabetes is a covered use under most commercial plans and under Medicare Part D. With commercial coverage, Lilly's savings card brings a fill to as little as $25, and the card caps savings at $150 a month and $1,950 a year across up to 13 fills. Patients whose commercial plan excludes Mounjaro can still use the card to pay $499. Medicare Part D members pay their plan's normal cost-sharing, and the Medicare GLP-1 Bridge does not apply, because the Bridge covers obesity prescriptions only.
The November 2025 deal set Medicare's price at $245. Lilly and the federal government agreed to a Medicare price of $245 a month for Mounjaro, Zepbound, Ozempic, and Wegovy across all doses and uses, and state Medicaid programs can get the same price. CMS said TrumpRx prices for injectable GLP-1s should fall toward that level over 24 months. Demand keeps climbing in the meantime: Mounjaro revenue reached $9.94 billion in the second quarter of 2026, up 91%, with $4.8 billion of that in the US. The FDA accepted two Sandoz applications for generic tirzepatide on June 29, 2026, but US patents run to 2036, so no generic launch date exists.

The Medicare GLP-1 Bridge, which started July 1, 2026, covers Foundayo, Wegovy, and the Zepbound KwikPen for obesity only, so Mounjaro sits outside it.
