In April 2026, Zepbound's WAC remains at $1,086/month but patients have multiple lower-cost options. LillyDirect cut vial prices in December 2025: starting doses at $299/month, maintenance at $399-$449/month. The multi-dose KwikPen (FDA-approved February 2026) offers a full month of treatment in a single pen at the same self-pay prices. TrumpRx.gov launched in February directing patients to these programs, and Medicare patients pay $50/month.
