PT-141 (bremelanotide) is a peptide that acts on melanocortin receptors in the brain to raise sexual desire. It is unusual among compounded peptides: it has an FDA-approved brand. That brand is Vyleesi, approved in 2019 for hypoactive sexual desire disorder (HSDD) in premenopausal women. Vyleesi is prescription-only.

Vyleesi pricing: Cosette Pharmaceuticals, which bought the drug from Palatin Technologies in 2024, makes Vyleesi. BlinkRx, a specialty pharmacy, is the only dispenser. The branded auto-injector lists at about $930 per 4-dose box, but manufacturer programs cut that sharply. Patients with commercial insurance pay $0 with a copay card, and uninsured patients pay $99 per box.

Compounded PT-141: Compounded PT-141 is widely prescribed off-label, meaning for uses FDA has not approved. That includes men with erectile dysfunction and low libido, which the Vyleesi approval does not cover. Since the February 2026 HHS announcement, pharmacies may compound PT-141 under the 503A and 503B pathways. A 503A pharmacy compounds patient-specific prescriptions; a 503B facility compounds in bulk for clinics. Compounded forms include injectable vials, nasal spray, sublingual troches (lozenges that dissolve under the tongue), and combination tablets such as bremelanotide plus tadalafil. The standard dose is 1.75 mg under the skin, taken 45 or more minutes before sex, with a cap of 8 doses a month.

Vyleesi (FDA-Approved, Branded)

The branded PT-141 product, dispensed only through BlinkRx specialty pharmacy as a prefilled 1.75 mg auto-injector. FDA approved it for HSDD in premenopausal women.
